Caught this 3” clamp just a couple of days ago…before it made it (back) into service.
Not sure it this is a casting flaw or happened during normal (ab)use
Guessing it’s a combination of those two.
Check your nuts.
Check your bolts.
Check your clamps.
Check your gaskets.
Check your flanges.
Regularly!
As if your life depended on it…
